Athletes
The athletic training staff is going to begin collecting your information online. This will mean LESS PAPERWORK for YOU!! Please follow the instructions below to help us in this process.
1. Click on this link (or copy and paste it into your web browser):
mwsu2.atsusers.com
2. Log on: Athlete ID: NEW
Password: NEW
(ALL CAPS)
Database: ATSMWSU (should already say that)
3. Fill out the first page COMPLETELY, anything with an astrix next to it
MUST be filled out in proper format in order to save, and
SAVE!!!
Athlete ID: (athlete choose his own username/ID, I’d suggest first and last name)
Password: (whatever the athlete will remember!!)
4. Other Tabs will appear at the top of the page after you SAVE.

Medical History Tab-- fill out any surgeries, etc. And click
SAVE!

Contact Tab fill out, and
SAVE!
7.
Athlete Forms Tab
- On the drop bar next to form name select “Medical History for Athletes”
- Click “New”
- Fill out
Medical History form completely. Every question
MUST be answered or athlete will not be able to save the form. At the end, if it will not save, you have forgotten and/or skipped a question. Once completed
SAVE!
8. Log out and you are finished!
